The face is one of the most important

The Procedure

There are 2 types of sculpting techniques,
open rhinoplasty and closed rhinoplasty. Open rhinoplasty means some or all
of the cuts for nose reshaping are made outside the nose. Closed rhinoplasty
means that all the cuts for reshaping are made inside the nose. The surgeon
will decide on which method at the patients medical evaluation
During surgery incisions are made either with a closed procedure inside the
nose or as an open procedure where an incision is made across the colummella
(the narrow strip of tissue that separates the nostril). Through these
incisions the soft tissues that cover the nose are raised to allow access to
reshape the structure of the nose. The nose is separated from its supporting
framework of bone and cartilage and then depending on the required results
the surgeon will remove some of the bone to reduce the hump and create a
slimmer bridge. If work is to be done on the tip of the nose, the cartilage
can be removed and the surgeon will remove small sections of the nose to
bring the nostrils closer together. Lastly by trimming the septum or
columella (the piece of skin that separate your nostrils) the angle of the
nose can be changed. When surgery is complete a splint will be taped to the
nose to ensure the bone and cartilage heal in the correct shape. |

After Surgery and Recovery |

For the first couple of days the area will be
swollen and bruised but this will subside. Your face will feel puffy, your
nose may ache and you may have a dull headache. Plan on staying in bed with
your head elevated for the first day. Applying cold compresses for the first
day will reduce the swelling and make you feel better. Most of the swelling
and bruising should disappear within 2 weeks or so. A little bleeding is
common during the first few days and you may feel some stuffiness for a few
days. Your surgeon will probably ask you not to blow your nose for the first
week or so while the tissue heals. If you have nasal packing it will be
removed after a few days.
By the end of week one, or occasionally, two weeks, all dressings, splints
and stitches should be removed.
Patients with glasses will have to wear contacts or prop them on their
forehead or cheeks.
Any strenuous activity should be avoided, (jogging, swimming, bending,
sexual relations – any activity that increases blood pressure) for 2-3 weeks.
Avoid hitting or rubbing your nose, or getting it sunburned for 8 weeks. Be
gentle when washing your face and hair and using cosmetics, make up can be
worn after 10 days. Patients may return to sedentary work a week or so
following surgery.
There are no visible scars with rhinoplasty except in the case of narrowing
of flared nostrils but even in this case the scars will fade and disappear
completely in a few months.
In the days following surgery the face may be bruised and swelling may come
and go often worsening in the morning, its easy to forget that the
appearance will improve. Many patients feel depressed after surgery – its
quite normal and understandable, this will pass. Day by day the nose will
begin to look better and gradual changes to the swelling, appearance of the
nose can be seen as it refines to a more permanent outcome. Within a week or
two you’ll no longer look as if you’ve just had surgery. The final results
of rhinoplasty may not be apparent for a year or more.
The results are permanent, although as aging continues the tip of the nose
becomes thicker and seems to look bigger in proportion to your deflating and
sagging cheeks. It is not uncommon to have rhinoplasty two or more times.
The nose is the central feature of the face and subject to a lot of scrutiny.

Risks and Complications of
Nose Surgery
Surgery is a serious business and not a decision to be taken lightly. There
is always the possibility that something may go wrong. No absolute
guarantees can be given. The surgeon will explain to you procedure, the
risks; alternatives and potential complications in detail so that you
understand what could go wrong.
It is very important that you follow the surgeons aftercare advice as this
is given to your for your benefit and will give you every chance of a speedy
and problem free recovery.
Complications of Nose surgery, when performed by a qualified surgeon are
infrequent and normally minor although there is a possibility of infection
or a reaction to the anaesthetic. To reduce any risk the surgeons
instructions should be closely followed both before and after surgery.
After surgery small burst blood vessels may occasionally appear as tiny red
spots on the skins surface and are usually minor, but permanent. There is no
visible scarring if the rhinoplasty is performed from inside the nose. There
may be small scars at the base of the nose that are not usually visible when
the procedure calls for the narrowing of flared nostrils.
In some patients rupture of the small surface vessels of the nose could
occur. Also poor wound healing. Cardiac and pulmonary complications can
occur in longer surgical procedures and may be associated with the formation
of, or increase in, blood clots in the venous system
Change in skin sensation (numbness) and nasal airway alterations may occur
after a rhinoplasty or septoplasty that may interfere with normal passage of
air through the nose
Nasal septal perforation (a hole in the nasal septum) may develop but is
rare; additional surgical treatment may be necessary to repair the nasal
septum but in some cases, it may be impossible to correct this complication.
Sutures may spontaneously surface through the skin, become visible or
produce irritation that require removal
Depression can often occur following cosmetic surgery. There are several
factors that contribute to post operative depression, including:
• The anticipation and stress of preparing for surgery can have a rebound
effect
• The medications, anaesthesia and trauma of the surgery have physical
effects that will affect your emotional well-being
• Stress and fear regarding complications and disfigurement
• Refraining from exercise can make you feel tired, blue, and out of sorts
• Adjusting to your new look
• Dealing with family and friends reactions to your surgery
• Financial stress
• Feeling guilty
Expecting to feel a bit down and accepting it as normal will help you get
back to normal sooner. Take time to relax, meditate and care for yourself.
